Insurance

I am a “fee for service provider.” This means I do not accept insurance or submit claims on behalf of patients, as insurance companies often dictate the type and amount of treatment you receive. If you have a PPO or POS plan you will likely be reimbursed some portion after you reach your deductible. However, every carrier is different. Please contact your provider to verify benefits prior to any appointments.

Fee

Cost per session is $565.00 for a 50 minute appointment or $315.00 for a 25 minute appointment.

Payment

I require a credit card on file for payment of services. Payment is processed after each appointment. At your request, I will send statements showing all charges and payments over the identified period of time. This statement will have all the necessary information needed to submit an insurance claim.

Cancellation and No Show Policy

Cancellations with less than 48 hours notice or No Show appointments will be billed at the full session fee unless:

  1. I can fill your appointment with another patient. I will make every effort to fill the time, and if I am able to, you will receive a credit or refund.
  2. Inclement weather prevents you and/or me from making it to the office safely. If this happens, a video or phone session will be scheduled if possible.
